What is Incision and Drainage (I&D)?

Incision and Drainage (I&D) is a surgical procedure performed to release and remove purulent or fluid collections from the body by making an incision. The primary purpose of I&D is to relieve pressure, eliminate infection, and promote healing.

This procedure is commonly performed for abscesses, infected cysts, hematomas, and other localized fluid collections involving the skin, subcutaneous tissue, or deeper structures.

From a medical coding perspective, documentation of depth, complexity, and anatomical site is essential for accurate CPT code selection.

What is Abscess ?

An abscess is a localized collection of pus caused by bacterial infection. It usually forms when the body’s immune response walls off an infection, resulting in a cavity filled with:

  • Pus

  • Dead tissue

  • Bacteria

Key coder points:

  • Abscesses may occur in the skin, subcutaneous tissue, muscle, or organs

  • Operative notes should specify location, size, and depth

  • Presence of loculations indicates increased complexity

​What is Hematoma?

A hematoma is a localized collection of blood outside blood vessels, usually caused by trauma or surgery. Unlike abscesses, hematomas are not infectious, but they may still require I&D if:

  • They are large

  • They cause pain or pressure

  • They fail to resolve spontaneously

Steps Involved in Incision and Drainage (I&D)

Although technique may vary, the typical steps documented in an operative report include:

  1. Patient positioning and site preparation

  2. Anesthesia administration (local, regional, or general)

  3. Incision made over the most fluctuant area

  4. Expression of purulent material or fluid

  5. Breakdown of loculations, if present

  6. Irrigation of the cavity

  7. Placement of packing or drain, if required

  8. Dressing applied

Important for coders:
The note should clearly state whether packing, drains, or extensive dissection were required.

Simple I&D

A simple I&D typically involves:

  • Single abscess

  • Minimal incision

  • No loculations

  • No packing or drain placement

  • Limited tissue involvement

Example documentation language:
“Single abscess incised and drained without loculations or packing.”

Complicated I&D ?

A complicated I&D involves one or more of the following:

  • Multiple abscesses

  • Loculations requiring breakdown

  • Extensive dissection

  • Packing or drain placement

  • Deep tissue involvement

Example documentation language:
“Multiple loculated abscesses were identified, broken down, irrigated, and packed.”
