What is hernia ?

Hernia is the protude of the internal part of the body such as interstine from the weak muscle or tissue wall of the abdomen.

Type of hernia

The type of hernia is based on the location and nature of hernia


Inguinal hernia : So, in this hernia occur at the inguinal canal. It means intestine part is protuding through inguinal canal to the download.

For male, the spermatic cord pass through inguinal canal.

For Female, round ligament passed through inguinal canal.


Umblical hernia : In this hernia, the intestine content protude from the umblical region of the abdomen or near the belly button.


Epigastric hernia : It is the hernia that occur at the epigastric region.


Femoral Hernia : It is the hernia that occur at the site of femoral canal.


Spigelian hernia : It is hernia in which instestine content protude to the spigelina fasica of the abdomen and it less common hernia.


Incisional hernia : In this hernia , the tissue protude from the previous surgical site because it become weak as compared to other location. So such type of hernia are incisional hernia.


What is abdominal hernia ?

This is the hernia in which internal instestine protudet throught the weak abdominal wall muscle , such as inguinal hernia, umblical hernia, femoral hernia , Spigelian hernia.


What is ventral hernia ?

It is hernia that occurs in the front of the abdomen , it means femoral and inguinal are not in the abdomen front region.

What is reducible and incarcerated hernia ? 

The hernia that can be reduced by pushing at the site of hernia is said to be reducible, if it is difficult to reduced then it said to be incarcerated hernia.


What is Strangulated hernia ?

If the protuded internal organs become choked or trapped sucha that blood supply get interrupted, leading to gangrene or tissue death, Such as hernia is called a Strangulated hernia


What is recurrent hernia ?

Recurrent hernia is the hernia that is not previously repaired .


What is Parastomal hernia ?

It is the type of hernia that forms next to stoma ( it is an artificial opening crated on the abdominal wall to remove waste


Direct and Indirect Hernia

  1. Direct Inguinal hernia is the protrusion of abdominal content/intestine through the posterior wall of inguinal canal.

  2. Indirect Inguinal hernia is the protrusion  of abdominal content/intestine into the inguinal canal via deep ring.


Hernia Sac

A hernia sac is a pouch formed by the inner lining of the abdomen, which contains the herniated contents such as intestine. 

What are the code selection Criteria for Hernia ?

  • Patient age

  • Encouter of hernia : Initial or recurrent

  • Nature of hernia: Incarcerated, reducible

  • Approach only for Inguinal hernia

Coding Guideline of Hernia

  1. The code is same for the Open and Laparoscopic repair of abdominal hernia

  2. No,need to report robotic guidance for repair of abdominal hernia as this inclusive.

  3. Implantation of mesh or other prosthesis is included and not need to code seperately.But for removal of infected mesh, use code 11008.For removal of non-infected mesh, report 49623 in conjunction with 49591-49622.

  4. Hernia defect size should be measured prior to opening the hernia defects.

Some important Interview Question


How will you report if you have multiple abdominal hernia that are next to each other and are touching and sharing the boundary ?

We will calculate the total size of the hernia defect by measuring from one end of the hernia to the other end. The small gap between the two hernias will also be included in the calculation.


How will report,  if two hernias are at 10 cm or greater distance and are separated by intact fascia ?

We will report the final size of the total defect by adding the sizes of the individual hernias and assign a single code


How will you report , if physician perform laparoscopy hernia repair of abdominal and also repair hernia at the site of umbilical port of laparoscope ? 

So as per CMS guideline, if a hernia repair is performed at the site of an incision for an open or laparoscopic abdominal procedure, the hernia repair is not separately coded.
