Indications for Surgery

The patient is a 65 year-old male with symptomatic recurrent bulbar urethral stricture causing weak stream, hesitancy, and incomplete emptying. Previous dilations provided temporary relief. The decision was made to proceed with Optilume balloon dilation for longer patency.


Description of Procedure

After informed consent, the patient was brought to the operating room and placed in the dorsal lithotomy position. Appropriate anesthesia was administered. The genital area was prepped and draped in the usual sterile fashion.


A well-lubricated 17 Fr cystoscope was introduced into the urethra. The urethra was inspected, and a short bulbar urethral stricture approximately 2 cm in length was identified. A sensor guidewire was advanced carefully through the stricture into the bladder under direct visualization.


The cystoscope was removed, leaving the guidewire in place.The appropriately sized Optilume drug-coated balloon catheter 50 mm diameter, 75 cm length was advanced over the guidewire and positioned across the stricture segment using both fluoroscopic and endoscopic guidance.


Once proper positioning was confirmed, the balloon was slowly inflated to the manufacturer-recommended pressure. Dilation was maintained for approximately 10 minutes to allow adequate mechanical expansion and delivery of paclitaxel into the urethral mucosa.

After the timed inflation, the balloon was deflated completely and removed. A repeat cystoscopic examination demonstrated a widely patent urethral lumen with no active bleeding.


A 16 Fr Foley catheter was placed for short-term postoperative drainage as per protocol. The patient tolerated the procedure well without complications.
