Digestive System for CPC Fresher


Digestive System


The Digestive System is responsible for:

  • Digestion of food

  • Absorption of nutrients

  • Elimination of waste


It consists of:

  1. Alimentary Canal (GI Tract)

  2. Accessory Digestive Glands

    • Salivary glands

    • Liver

    • Gallbladder

    • Pancreas


The alimentary canal begins at the mouth and ends at the anus.


Oral Cavity

The oral cavity (mouth) contains the teeth and the tongue.

Adults have 32 permanent teeth, including 8 incisors, 4 canines, 8 premolars, and 12 molars, with a dental formula of 2-1-2-3 / 2-1-2-3. The hard outer covering of the tooth is called enamel, which helps in chewing (mastication).

Pharynx

The pharynx serves as a common passage for both food and air. During swallowing, a cartilaginous flap called the epiglottis closes the opening of the trachea, preventing food from entering the airway.

The pharynx is a muscular tube that connects the oral and nasal cavity to the larynx and esophagous.

Esophagus

The esophagus is a muscular tube that carries food from the pharynx to the stomach by peristalsis. A gastroesophageal (lower esophageal) sphincter regulates the passage of food into the stomach and helps prevent acid reflux.


The esophagus has two sphincters that regulate the movement of food and prevent reflux.

  1. The Upper Esophageal Sphincter (UES) is located between the pharynx and the esophagus. It opens during swallowing to allow food to enter the esophagus and closes afterward to prevent air from entering the digestive tract and food from moving back into the throat.

  2. The Lower Esophageal Sphincter (LES), also called the Gastroesophageal Sphincter or Cardiac Sphincter, is located between the esophagus and the stomach. It relaxes to allow food to enter the stomach and then closes to prevent stomach acid and food from refluxing back into the esophagus.

Dysfunction of the LES commonly leads to Gastroesophageal Reflux Disease (GERD).


Stomach

The stomach is a J-shaped muscular organ located in the left upper quadrant (LUQ) of the abdomen. It has four parts: cardia, fundus, body, and pylorus. Food leaves the stomach through the pyloric sphincter into the duodenum, the first part of the small intestine.

In the small intestine, it forms villi, which greatly increase the surface area for nutrient absorption.

Small Intestine

The small intestine is the longest part of the digestive tract and is the primary site for digestion and nutrient absorption.

It has three parts: the duodenum, which receives bile and pancreatic enzymes; the jejunum, where most nutrient absorption occurs; and the ileum, which absorbs vitamin B12 and bile salts before joining the large intestine.


Large Intestine

The large intestine consists of the cecum, colon, rectum, and anal canal.

The cecum is the first part of the large intestine and gives rise to the vermiform appendix, a finger-like vestigial organ.

The colon is divided into four parts: ascending, transverse, descending, and sigmoid colon. The primary functions of the large intestine are water absorption, electrolyte absorption, and formation of feces.

Rectum

The rectum stores feces until defecation, and the anus is the terminal opening through which feces are expelled.

The wall of the alimentary canal from the esophagus to the rectum is composed of four layers: serosa, muscularis, submucosa, and mucosa.

  1. The serosa is the outer protective covering.

  2. The muscularis contains smooth muscle responsible for peristalsis.

  3. The submucosa contains connective tissue, blood vessels, lymphatic vessels, nerves, and glands.

  4. The mucosa is the innermost layer responsible for secretion and absorption.

The mucosa undergoes several modifications to improve digestive function.


Accesory Gland of digestive System

The accessory digestive glands assist in the digestion of food by producing digestive secretions. Although food does not pass through these organs, they play an essential role in the digestive process.

The major accessory digestive glands are the salivary glands, liver, gallbladder, and pancreas.


Salivary gland

The salivary glands produce saliva, which moistens food, lubricates the oral cavity, and begins the digestion of carbohydrates through the enzyme salivary amylase (ptyalin). Humans have three pairs of salivary glands:

  • Parotid glands – located in the cheek, in front of the ears; the largest salivary glands.

  • Submandibular (Submaxillary) glands – located beneath the lower jaw.

  • Sublingual glands – located beneath the tongue.


Liver and Pancreas

The liver is the largest gland in the human body, weighing approximately 1.2–1.5 kg in adults. It is located in the right upper quadrant (RUQ) of the abdomen, just below the diaphragm, and consists of right and left lobes.



The liver produces bile, which helps in the digestion and absorption of fats.

Bile is transported through the right and left hepatic ducts, which join to form the common hepatic duct. It is then stored and concentrated in the gallbladder, a small muscular sac located beneath the liver.


The cystic duct from the gallbladder joins the common hepatic duct to form the common bile duct (CBD). The common bile duct joins the pancreatic duct and opens into the second part of the duodenum through the hepatopancreatic ampulla (Ampulla of Vater). The opening is regulated by the Sphincter of Oddi, which controls the flow of bile and pancreatic juice into the duodenum.

The Ampulla of Vater (Hepatopancreatic Ampulla) is a small chamber formed by the union of the Common Bile Duct (CBD) and the Main Pancreatic Duct. It opens into the second part of the duodenum, allowing bile and pancreatic juice to enter the small intestine for digestion.


Sphincter of Oddi

The Sphincter of Oddi is a ring of smooth muscle located around the opening of the Ampulla of Vater. It regulates the flow of bile and pancreatic juice into the duodenum and prevents the backflow of intestinal contents into the bile and pancreatic ducts.



The pancreas is an elongated gland located behind the stomach and within the C-shaped curve of the duodenum. It is a mixed (heterocrine) gland because it has both exocrine and endocrine functions.


The exocrine pancreas secretes alkaline pancreatic juice containing digestive enzymes such as amylase, lipase, trypsin, and chymotrypsin, which aid in the digestion of carbohydrates, fats, and proteins.

The endocrine pancreas, through the Islets of Langerhans, secretes the hormones insulin and glucagon, which regulate blood glucose levels.
